RECOVERY NETWORK INC. — CONSENT FOR AI-ASSISTED AUDIO RECORDING

I, the undersigned patient, understand and consent to the following:

1. Purpose. Audio recording of this clinical session will be used to generate automated clinical documentation (DAP notes) via Recovery Network's AI Audio Scribe, assisting the clinician in accurate and timely record-keeping.

2. AI Processing. The recording will be processed by The Council (Dr. Rivera — Documentation) to generate a draft clinical note. The treating clinician will review, edit, and sign all generated documentation before it becomes part of my medical record.

3. Data Protection. All recordings are encrypted in transit and at rest under HIPAA §164.312 technical safeguards. Substance use disorder records are additionally protected under 42 CFR Part 2 and will not be disclosed without separate written authorization.

4. Retention. Audio recordings are retained for 72 hours for quality assurance and then permanently deleted. Only the signed clinical note is retained as part of the medical record.

5. Right to Refuse or Withdraw. I may refuse recording at any time without impact on my treatment. I may request recording be stopped during the session and any captured audio be deleted.

6. Compliance Veto. Dr. Chen (Compliance Veto Authority) validates the consent chain before any recording begins. If consent is incomplete or invalid, the recording system will not activate.
